Per state guidelines, this position requires a 4 -year degree in Social Services or related field OR 4-year degree in unrelated field plus one year of social work/care coordination. With each client, you will work to uncover their needs and aspirations, assisting to remove any barriers that get in the way through connecting them with services and resources.
