About The Position

Per state guidelines, this position requires a 4 -year degree in Social Services or related field OR 4-year degree in unrelated field plus one year of social work/care coordination. Requirements

  • To be eligible to provide AC/BI/CADI/EW Case Management you must have: Bachelor’s degree in health and human services OR bachelor’s degree in any field with 1-year of experience in the human services field
  • To be eligible to provide DD Case Management you must have: Bachelor's degree in social work, Special Education, Psychology, Nursing, Human Services or other related fields AND One year of direct experience working with individuals with developmental disabilities or related conditions OR have taken a course with a focus on developmental disabilities
  • Ability to pass DHS background check
  • Maintain Driver’s license & vehicle insurance

Responsibilities

  • Complete new client intakes
  • Develop initial service plan with the client and legal representative
  • Coordinate services between providers and clients
  • Schedule in-person meetings with clients at least twice per year and make monthly phone contact
  • Maintain quality case notes documenting service plans, progress on goals, service agreements, etc.
  • Attend first week of onboarding training, as well as quarterly trainings, at our headquarters in St. Paul (Per diem and mileage reimbursement provided)
