About The Position

This position is responsible for coordinating eligibility and assessments for WA Cares Beneficiaries in need of long-term care. Key responsibilities include conducting in-depth assessments, assisting with applications, providing referrals to local and state programs, building community partnerships, and managing program information. The role requires strong knowledge of long-term care, social service networks, and computer skills.

  • BA in social services or related field and two years of paid or volunteer experience providing direct human services or two years of relevant college-level courses and four years paid or volunteer experience providing direct human services.
  • This position must obtain Basic Life Support (BLS) CPR within 90 days of hire date and is required to maintain current BLS CPR throughout employment.
  • Valid WA State Driver’s License and proof of liability insurance.
  • Pre-hire and annual employee health screening required.
  • Annual influenza vaccine required.
  • COVID-19 Vaccination is a mandatory condition of employment.

  • Other language skills may be considered depending on site needs due to the population that is being served.

Responsibilities

  • Participate in team phone calls or meetings to anticipate the beneficiaries’ clinical, social, and behavioral health needs.
  • Work with the WA Cares team to identify gaps in care and work to resolve them using process improvement strategies.
  • Provide information to enable beneficiaries to age in place, with services like in-home personal care, home modifications, home-delivered meals, and more.
  • Connect patients to Sea Mar and non-Sea Mar resources as appropriate and track all resources available to patients. These services may include, but are not limited to, insurance enrollment, preventive health services, behavioral health, dental, and care management.
  • Assist beneficiaries with ongoing self-management goal setting based on mutual goal setting and with emphasis on client decision-making utilizing motivational interviewing skills.
  • Follow up with beneficiaries to evaluate their condition and address barriers to the care plan.
  • Disseminate information regarding the services provided to the beneficiaries to improve their quality of life for their physical, mental, and emotional well-being while participating in the WA Cares Program.
  • Exhibit excellent customer service skills by using active listening skills, greeting clients in a welcoming manner, making them their only priority when providing services, and assisting in meeting their needs.
  • Ensure accurate and timely assessments.
  • Work with other staff to guarantee that all assessments are completed accurately and within the required timeframe.
  • Conduct record reviews to ensure that assessments, service plans, and case documentation are complete and accurate.
  • Manage the assignment of cases and workload distribution to ensure efficiency and equitable distribution of work.
  • Participate in inviting clients to set up their Follow My Health account and provide brief information about its benefits.
