About The Position

The Vocational Rehabilitation Transitional Coach is responsible for the completion of activities associated with the Vocational Rehabilitation grant’s objectives and deliverables. The Vocational Rehabilitation Transitional Coach will serve clients with disabilities and will be responsible for connecting and staying engaged with the client and their family as needed.

Requirements

  • Bachelor’s degree in Rehabilitation Counseling, Human Services, or related field.
  • Must have at least two (2) years of experience providing case management to Native Americans with disabilities.
  • Must be competent in Tribal, State and Federal regulations and law as they apply to American Disabilities Act (ADA) and Vocational Rehabilitation Services.
  • Must have strong interpersonal and communication skills, including the ability to communicate effectively, orally and in writing, through concise reports, memoranda, directives, emails, and letters.
  • Must have the ability and interest to greet the public and handle difficult situations on and off the phone in a professional and courteous manner. Including the ability to work with angry or disgruntled clients in a cooperative and effective manner.
  • Ability to always maintain strict confidentiality.
  • Must have communication skills, both oral and written, to interact with staff, clients, officials, funding agencies, medical staff, and other individuals regarding program and client matters.
  • Be computer literate and able to operate software for letter processing and spreadsheets.
  • Must be of good moral character.
  • Will be required to pass Federal background checks and submit to pre-employment drug testing.
  • Must possess a valid driver's license, maintain valid insurance, and be insurable under the Tribe's insurance policy.
  • Proven ability to work well with Indian families and must have positive interaction skills.

  • Must possess the ability to read, write, and speak the English language fluently.
  • Must be able to employ professional verbal and written communications skills continually and effectively.
  • All applicants will comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D, which require all prospective employees to sign a declaration prior to employment regarding all arrests and convictions of child abuse or violent felonies and to comply with PL 101-630 and PL 101-637 regarding criminal records check.

Responsibilities

  • Demonstrates the Winnebago Tribe’s core mission, vision, and values.
  • Maintains a positive and helpful relationship with clients, Tribal staff, partners and stakeholders and employers.
  • Assisting walk-ins, and over-the-phone requests for services, and coordinating referrals from other agencies.
  • Utilizes knowledge of principles and practices of vocational rehabilitation counseling to determine eligibility.
  • Consults with physicians, psychologists, and other disability professionals regarding the functional limitations, treatment and prognosis of eligible applicants.
  • Assists clients to develop their Individualized Plan for Employment (IPE).
  • Provides vocational counseling and guidance in the effective delivery of case services.
  • Participates in job readiness, job search, and job placement activities to ensure a successful case outcome for the client.
  • Follows program record documentation guidelines, using proper maintenance of clients’ case files and maintains documentation in the program database.
  • Arranges assessments, secures transportation, and transports clients to assessments when needed and when making referrals.
  • Supports case management for clients: including but not limited to setting appointments and home visits.
  • Attends in/out of state travel/training
