Liberty Resources is seeking a Full-Time Vocational Case Manager with our DePalmer House program located in Syracuse, NY. Requirements

  • Bachelors/Associates Degree and two (2) years’ related experience in vocational services.
  • Must possess a valid New York State Driver’s License and have access to reliable transportation.
  • Strong organizational and interpersonal skills.
  • Ability to develop and promote employment programs with area employers and service providers.
  • Ability to work successfully within outcome-driven program.
  • Demonstrates initiative and sound judgment.
  • Strong written and verbal communication skills necessary.

Responsibilities

  • Completes Intake Assessments for all residents of DePalmer House.
  • Participates in the development of an individualized service plan for assigned clients; coordinates and facilitates the delivery of services in accordance with service plan.
  • Understands and applies case management principles to increase DePalmer House resident self-sufficiency and transition from unemployment to competitive employment.
  • Responds effectively to client emergencies, which may require flexibility of both time and areas of assignment.
  • Establishes relationships with and utilizes community service providers and employers for a broad range of client needs, including referral to housing, medical care, addiction treatment, mental health counseling, educational and vocational services.
  • Completes all required documentation, in timelines in accordance with program standards.
