About The Position

Tarlani Healthcare is seeking a Visiting RN for a Home Health Agency in Oxnard, CA. This role involves providing high-quality medical care to patients in their homes, ensuring they maintain their independence. The RN will be responsible for administering treatments, conducting assessments, and coordinating care with other healthcare professionals. The position offers flexible scheduling options and requires a commitment to patient care seven days a week, with a minimum availability of two to three days each week.

Requirements

• Current CA RN License (Required)
• BLS from the American Heart Association and/or the American Red Cross (Required)
• IV/Phlebotomy Certified (Required)
• 1 year of experience in Home Health or Hospice (Required)
• Comprehensive knowledge of OASIS documentation (Preferred)

Responsibilities

• Providing nurse visits to home health patients (home visits).
• Performing an initial, comprehensive assessment.
• Documenting everything and uploading to the Agency Portal.
• Obtaining and documenting physician orders.
• In consultation with the assigned Clinical Team Manager, initiating and regularly re-evaluating and revising the plan of care.
• Coordinating care with the assigned Clinical Team Manager, physician, and other members of the home care team, informing them of significant changes in the patient's condition and needs.
• Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager.
• Completing and submitting all related documentation.
• Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA).
• Performing Home Health Aide supervisory visits per regulatory requirements.
