About The Position
The Visiting Nurses Association (VNA) of Manchester and Southern New Hampshire is dedicated to providing exceptional in-home healthcare services to patients in Manchester and surrounding communities. Our mission is to support patients as they navigate the challenges of recovery from surgery, manage life-limiting illnesses, or cope with physical disabilities. We pride ourselves on delivering individualized care that brings peace of mind to both patients and their families, ensuring that professional healthcare services are accessible in the comfort of their homes. As a Licensed Nursing Assistant (LNA), you will play a crucial role in our care team, performing delegated nursing and homemaking duties for patients of all ages. Your responsibilities will include conducting PT/INR checks as ordered by physicians, documenting patient needs and responses, and reporting any changes in patient conditions to the appropriate case or clinical manager. You will adhere to the care plans established by primary nurses or therapists, ensuring a safe and healthy environment for our clients. In addition to providing direct care, you will reinforce the education provided by primary nurses or therapists by reviewing personal care techniques with caregivers and patients. You will also be responsible for maintaining accurate clinical records, time sheets, and quality control logs, and you will be expected to attend a minimum of 12 hours of in-service training each year. Your role is vital in promoting patient dignity and creating a calm, supportive, and therapeutic environment for clients and their families.
