About The Position

The Home Health Aide (HHA) is a vital member of the home care team, working under the supervision of a registered nurse or therapist. This role involves providing personal care services to patients in their homes, ensuring their comfort and safety while promoting quality and continuity of care. The HHA is responsible for observing and documenting patient conditions and care provided, making it essential for maintaining effective communication with the healthcare team.

Requirements

• Meets training requirements in accordance with State and Federal laws.
• At least 18 years of age.
• Ability to read and follow written instructions and document care given.
• Self-directing with the ability to work with little direct supervision.
• Empathy for the needs of the ill, injured, frail, and impaired.
• Possess and maintain current CPR certification.
• Demonstrates tact, patience, and good personal hygiene.
• Licensed driver with an insured automobile.
• Must have a valid and active HHA license.

Responsibilities

• Providing personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting patients with dressing and undressing, skin care to prevent breakdown, assisting with toileting activities, and keeping the patient's living area clean.
• Planning and preparing nutritious meals for patients.
• Assisting in feeding the patient when necessary.
• Taking and recording vital signs such as temperature, pulse, respiration, and blood pressure as ordered.
• Assisting in ambulation and exercise according to the care plan.
• Performing range of motion and other simple procedures as ordered.
• Assisting patients in the self-administration of medication.
• Doing the patient's laundry as appropriate.
• Meeting safety needs of patients and using equipment safely and properly.
• Reporting on the patient's condition and significant changes to the assigned nurse.
• Adhering to documentation and care procedures and standards of conduct.
