About The Position

Bring Compassionate Care Home – Join the ProHealth Home Health & Hospice Team Schedule Details: This .7 FTE commits to 28 hours per week. 3-4 days/week rotation 8:00 am - 4:30 pm, every third weekend, holiday rotation. Our team is recognized for excellence in clinical outcomes, personalized care, and collaborative service delivery — making us a top choice not just for patients, but for skilled, compassionate nurses looking for meaningful, flexible work in a supportive environment.What You Will Do:As a Visit RN, you’ll provide hands-on care to patients in their homes—delivering skilled nursing care and making clinical decisions that directly impact a patient’s health and independence. Whether developing or following a plan of care, you’ll work collaboratively with case managers, physicians, social workers, and family caregivers to support optimal health outcomes. This role is ideal for a nurse who is a strong communicator, critical thinker, and teacher—someone who enjoys working autonomously and appreciates the reward of caring for one patient at a time in a meaningful and personal way.

Requirements

  • Associate’s Degree in Nursing required; Bachelor’s Degree in Nursing preferred
  • Current RN license in Wisconsin or through an eNLC compact state
  • Valid WI driver’s license and acceptable driving record
  • BLS certification (or ability to obtain upon hire)
  • Strong communication, problem-solving, and time management skills

Nice To Haves

  • Home health or hospice experience preferred
  • Experience with EPIC preferred

Responsibilities

  • Deliver individualized nursing care to home health or hospice patients
  • Develop, follow, or revise a patient’s plan of care based on ongoing clinical assessments
  • Provide patient education, medication management, and wound care
  • Collaborate closely with case managers, physicians, and interdisciplinary teams
  • Use EPIC to document visits, track progress, and coordinate services
  • Empower patients and families to manage health needs in the home environment
