Compensation is based on total years of experience in previous clinical roles. In this role, you'll care for patients in their homes in the Stillwater Service Area of Allen, Huntington, Jay, Blackford, Grant, Wells, Whitley, Wabash, Kosciusko, Noble, DeKalb, and Adams Counties. Both RN and LPN are encouraged to apply. You will be working as Needed (PRN), which may include Monday through Friday, Weekends, or Holidays. Requirements

Graduate of an accredited school of nursing.
Must have a current RN license to practice in the state of Indiana.
Must hold current CPR certification or obtain within the orientation period.
Must hold current TB certification or obtain within the orientation period.
Must be able to provide those services requiring substantial specialized nursing skill.
Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
Must be able to drive independently to all patient/family clients’ homes.
Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of him/her, or to others in the workplace.
Must be comfortable working with hospice patients, with the emphasis on palliative care.
Ability to recognize needs and emergencies of hospice and palliative care patients, and provide appropriate interventions.
Strong interpersonal skills.
Strong assessment skills.
Must have visual and hearing ability to comprehend written and verbal communication. Constant.
Must be able to engage in clear, concise verbal/nonverbal communication. Constant.
Must have the ability to remain calm in occasional emergency situations.
Recognizes emergencies and has knowledge of emergency procedures, according to Agency policy.
Must be willing to accept assignments without regard to location, socioeconomic environment, disease, or ethnic origin.
Must exhibit dependability in areas such as attendance, tardiness, and timely performance of duties.
Must be willing to make scheduled visits on holidays, weekends, or evenings when assigned.

Nice To Haves

Bachelor’s degree in Nursing preferred.
Infusion skills preferred
Prefer one (1) year of recent hospital or home health care experience.

Responsibilities

Performs nursing procedures and care to assigned terminally ill patients by assessing needs and implementing nursing care plans under the coordination of the RN Case Manager.
Must be capable of functioning independently in the patient's setting within the scope of the Registered Nurse.
Implements Agency policies in all work-related situations; orientation of patient/family to hospice benefit services, including identifying the role of team members.
Follow state and agency policy and procedures when legally pronouncing a death. Specifics of the death event will be documented in the clinical record; required information is communicated/submitted to appropriate parties. The patient’s body will be handled with respect and dignity and in accordance with individual patient/family requests.
Respect the patient's or family's cultural and religious traditions and beliefs.
Recognize and maintain professional boundaries in relationships with the patient/family/caregiver.
Documents findings accurately, in a timely manner, and on appropriate forms, in accordance with the Agency's procedures.
Delivers nursing care to patients, which includes pain/symptom management, teaching, providing direct care, case management, referral, advocacy, and monitoring.
Supervises clinically the care delegated to LPNs, Home Health Aides, CNAs, Companions, and/or Homemakers.
Instructs LPNs, Home Health Aides, and/or CNAs on specific patient care procedures within approved policies.
Maintains a therapeutic relationship with patients/families. This includes the confidentiality of personal matters, patients', coworkers', and the Agency's business.
Follows orders from the patient’s physician and informs the physician and other personnel of changes in the patient’s condition and needs.
Demonstrates understanding of hospice philosophy and concept of care and considers the patient/family as a unit of care.
Assists with the preparation, implementation, and continuing evaluation of the Interdisciplinary Plan of Care.
Provides for the emotional and physical comfort and safety of patients. Adheres to Agency policies in all work-related situations.
Observes, records, and reports to the appropriate person symptoms, reactions, and changes, including: 1. General physical and mental condition of patients, and signs and symptoms that may indicate adverse changes. 2. Stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors.
Orients patient/family to hospice services, including identifying the role of the team member.
Anticipates issues related to the patient's impending death and pursues resolution.
Remains flexible for patient coverage and accepts responsibility under adverse conditions, such as weather-related crises.
Maintains expected and acceptable productivity.
Works in a cooperative relationship with other Agency personnel.
Seeks appropriate clinical supervision.
Participates in the Agency's committees and task forces.
Assumes responsibility for continued personal growth and development.
Participates in on-call rotation as assigned.
Seeks educational opportunities to improve clinical practice and participates in in-service programs.
Adheres to all the Agency's policies.
Any other duties as assigned by the supervisor.
Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.
Participates in quality improvement activities and/or other agency committees or task forces when requested.
Proficient and timely in all aspects of clinical documentation, including computer data entry.
Proficient in blood draws, all routine patient treatments (dressing changes, catheters, etc.), pain and symptom management, care of the patient and family, including pediatric and HIV + patients; Mantoux administration.
Proficient in all aspects of IV care, including programming of IV pumps, tubing changes, site care, and subcutaneous access.
