About The Position

Concierge Home Care is growing, and we are actively hiring a Remote Virtual Care Physical Therapist Assistant (PTA) to join our team!

What is a Virtual Care Physical Therapist Assistant (PTA)? 

A Virtual Care PTA provides support to the Complex Care Team Program at Concierge Home Care. A Virtual PTA participates and collaborates with the clinical team for oversight of patients identified as high risk for hospitalization.

Requirements

• Florida Physical Therapist Assistant License (Required)

Nice To Haves

• CPR Certification (Preferred)
• Microsoft Office preferred

Responsibilities

• Performing virtual visits
• Assessing psychosocial needs
• Monitoring telehealth
• Facilitating patient needs across the continuum of care
• Providing education concerning relevant medical diagnosis and factors that would increase the risk for hospitalization
• Data tracking
• Attending required rehospitalization-related meetings, conferences, and symposiums throughout our agency and communities
• Tracking data, identifying hospital admission trends, and providing patient and staff education to prevent hospitalizations
• Assisting with innovation and implementation of protocols and guidelines for Concierge Best Practices Programs
• Manage virtual care and telehealth systems by providing virtual assessments, education, and data tracking
