About The Position

The Veterans Engagement Specialist at Transition Projects Inc. is responsible for providing comprehensive engagement and outreach services to veterans and their households who are experiencing homelessness or are at imminent risk of homelessness. This role involves coordinating with the SSVF Case Management Team to assist participants in accessing shelter, housing, and necessary support services, while also developing individual goals to help them achieve stability and self-sufficiency.

Requirements

  • Bachelor's degree in a related field or 2 years of experience in social services.
  • Demonstrated ability to interact with other agencies and advocate for participants.
  • Knowledge of chemical dependency and mental health issues and treatment resources.
  • Ability to work effectively with property and building managers.
  • Ability to provide mediation between agencies and participants.
  • Familiarity with community resources for the homeless and veterans.
  • Ability to work as part of a team in service delivery.
  • Computer literacy and ability to maintain accurate participant files.

Nice To Haves

  • Bilingual in English and Spanish preferred.
  • Experience as a veteran is a plus.

Responsibilities

  • Assist in the development and implementation of individual goals and objectives for veterans to attain stability and housing.
  • Provide engagement and outreach to veterans through various communication methods including phone, email, and field visits.
  • Work with referral sources and community partners to identify veterans needing assistance.
  • Complete screening assessments to determine eligibility for veteran services.
  • Provide education and resources to participants regarding their specific needs and available services.
  • Address barriers to individual goals including mental health, substance use, and income development.
  • Coordinate with agency staff to ensure consistent and comprehensive services to participants.
  • Develop and maintain relationships with community resources to assure service availability.
  • Educate community partners about the Veteran Services programs and eligibility determinations.
  • Maintain participant confidentiality and adhere to relevant regulations.
