About The Position

VETERAN SERVICES CASE MANAGER SPECIALIZED SERVICES FULL TIME – NON-EXEMPT Thrive Behavioral Health, the Certified Community Behavioral Health Clinic for Kent County, has been delivering mental health and substance use services since 1976. Our programs include: Adult Mental Health, Youth and Family, Substance Use, Housing, Teen and Young Adult, Court Ordered, and Crisis Behavioral Health. With more than a dozen locations across Rhode Island and more than 300 employees, Thrive is continuing to grow and meet the needs of the community. General Summary: There is need for specialized services for Veterans and their families such as trauma informed care, housing services and case management. The Veteran Services Case Manager will travel to all Thrive locations to work with Veterans and their families and make referrals to appropriate services. In addition, this position will provide outreach services in the community and attend veteran-based functions in the support of Veterans and their community in the Kent County Area. Schedule: Mon-Fri, First Shift Location: Warwick, RI Salary: Starting at $25/hr, depending on experience. Requirements

  • A minimum of an Associate’s degree; in social/health sciences, social work, sociology, psychology, public administration, or related discipline.
  • A minimum of 4 years’ experience conducting community outreach and coordination while working with active military or veterans and their families or minimum of 4 years of honorable service in the armed forces.
  • Demonstrated knowledge and experience working in the community sector with diverse organizations and with individuals living with a range of diverse needs including disabilities, mental health, cultural diversity, and complex care needs.
  • Demonstrated ability to consult and liaise with health and community organizations to coordinate activities.
  • Demonstrated high level of verbal, written, and interpersonal communication skills.
  • Working knowledge and proficiency in Microsoft Office applications.
  • Valid driver's license and automobile insurance which meets Rhode Island minimum standards , or the ability to obtain necessary transportation to perform the responsibilities/tasks of the job.
  • If still serving in a National Guard or Reserve capacity, a valid and updated Military ID is required.

Responsibilities

  • Review community needs assessment information to identify barriers to access for veterans and their families.
  • Build effective partnerships to facilitate engagement and outreach and promote grassroots marketing initiatives.
  • Supporting Veteran Services Coordinator in program development and community outreach.
  • Coordinate referrals and linkages to community resources.
  • Coordinate services within the Organization as needed and complete all necessary follow-ups.
  • Assist clients in developing and strengthening their support network to accomplish goals and objectives for veterans and their families
  • Collaborate with other providers, agencies and individuals in the client network of care.
  • Maintain a working knowledge of community resources and current trends in veteran services.
  • Perform general administrative duties as assigned.
  • Carry and enliven a positive Thrive image consistent with the Thrive’s Guiding Principles and professional code of ethics.
