St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St. Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals. Position SummaySt. Patrick Center, a ministry of Catholic Charities, is hiring a Rural Counties Case Manager to work in the Supportive Services for Veteran Families (SSVF) program. As a member of the SSVF team, this case manager will use best practices to assess the housing needs of veterans who are unhoused or at risk of becoming unhoused in Franklin, Jefferson, St. Francois, and Washington counties. The case manager will utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability by meeting with the veteran families to develop an individual treatment plan, coordinate individualized services for housing and financial stability, and document all support related to activities and services. This position will primarily be held in our St. Charles, MO, location and cover St. Charles, Lincoln, and Warren counties. We require a reliable vehicle since the position requires the case manager to meet clients regularly in several counties. The core working hours for this position are Monday through Friday 8:00 am to 4:30 pm.
