Veterans with prior case management experience are strongly encouraged to apply for this role. The Patriot Place Coordinator will be responsible for guiding and supporting veterans through their journey at Patriot Place by providing direct case management services, peer mentoring, and facilitating various program components. This includes conducting assessments, assisting in the development and ongoing evaluation of care plans, and ensuring veterans have access to necessary services. The role requires building strong relationships with veterans to foster self-sufficiency, personal growth, and overall well-being. Case management is a crucial component of the Patriot Place model, and the team’s mission is to advocate for veterans as they transition out of homelessness and into a new chapter of stability and independence. This role will provide critical support to veterans as they adjust to stable housing. Patriot Place is a six-unit apartment complex specifically designed to serve veterans who were previously experiencing homelessness. Located in St. Johns County, this permanent housing project provides not just shelter, but a supportive environment where veterans can rebuild their lives. Patriot Place is a collaborative effort between UF Health St. Johns and St. Johns Housing Partnership, reflecting a shared commitment to ending veteran homelessness through stable housing, supportive services, and community-based partnerships. The coordinator will play a key role in ensuring the success of this initiative by delivering compassionate, client-centered support to the residents of Patriot Place.
