About The Position

The Transitional Housing Case Manager provides comprehensive, trauma-informed case management services to veterans participating in the Transitional Housing Program. The Case Manager supports veterans in achieving housing stability, increasing income, improving wellness, and transitioning to permanent housing. This position requires a strong understanding of veteran-specific resources, including Veteran Affairs systems of care and community-based support services. This position manages referrals through the Coordinated Entry System (CES) and walk-ins into the Transitional Housing Program.

Requirements

  • AOD Registered and/or Certified, or BA/BS in related field, preferred
  • Basic understanding of the military and the Veterans Administration
  • Minimum of a Social Work or related field preferred
  • Minimum of 6 months of paid case management experience in related field, preferred.
  • Ability to prioritize competing responsibilities and thrive in challenging situations
  • Able to change priorities at a moment's notice
  • Must be able to fluently read, write and communicate in English
  • Must be able to comfortably, both mentally and physically, perform all aspects of job description and other duties as assigned by supervisor
  • Ability to bend and lift light objects
  • Ability to sit for extended periods of time while viewing a computer monitor
  • Dexterity of hands and fingers to operate a computer keyboard
  • Capable of hearing and speaking to exchange information
  • Seeing to read reports and data
  • Must be able to work in a noisy, busy environment

Nice To Haves

  • Prefer prior experience working with homeless population
  • Knowledge of HMIS databases a plus!

Responsibilities

  • Conduct comprehensive intake assessments to identify barriers to housing stability, income, healthcare, and overall well-being
  • Develop individualized service plans in collaboration with each veteran, incorporating short- and long-term goals
  • Provide ongoing weekly case management through regular one-on-one meetings and progress evaluations
  • Assist veterans in securing permanent housing and maintaining stability using a Housing First approach
  • Connect veterans to VA benefits, healthcare services, mental health treatment, substance use services, and other veteran-specific resources
  • Support veterans in obtaining employment, education, vocational training, and income benefits (VA compensation, SSI/SSDI, etc.)
  • Assist with obtaining identification documents, discharge paperwork (DD-214), and other required documentation
  • Conduct routine unit inspections to ensure compliance with program guidelines
  • Provide crisis intervention and de-escalation when needed
  • Maintain accurate, timely documentation in case management systems in compliance with agency requirements
  • Collaborate with community partners, landlords, and internal staff to coordinate care
  • Participate in case conferencing and multidisciplinary team meetings
  • Coordinates with potential clients and care team to start application and vetting process with CES referral system.
  • Coordinate treatment with appropriate Therapist, employment counselor, and other VVSD staff on a regular basis as appropriate and/or as directed by supervisor(s).
  • Review assigned case files weekly.
  • Review/authorize client passes.
  • Encourage clients to attend all appointments as appropriate.
  • Work as a team with other staff on client interventions when appropriate.
  • Other duties as assigned by supervisor.
