The VAD/Heart Transplant Coordinator will be responsible for the coordination of care for all patients requiring implantation of a cardiac assist device. These include patients accepted for, and awaiting heart transplant, or those with Advanced Heart Failure in which the device is implanted as a permanent treatment option. The VAD/Heart Transplant Coordinator will also be responsible for the coordination of care for all patients who are pre- or post- heart transplant. The VAD/Heart Transplant Coordinator will work as a member of the Advanced Heart Failure Team, and will be the primary liaison with the transplant cardiologists and cardiac transplant surgeons, other physicians, and health care staff throughout the hospital. A major component of the position is patient and family education, daily rounds, discharge planning, and on-call coverage. The VAD/Heart Transplant Coordinator is also responsible for outreach activity to promote the programs, frequent communication with referring physicians, and professional education. The VAD/Heart Transplant Coordinator functions within the parameters outlined in the LVAD practice guidelines.
