About The Position

The VAD/Heart Transplant Coordinator will be responsible for the coordination of care for all patients requiring implantation of a cardiac assist device. These include patients accepted for, and awaiting heart transplant, or those with Advanced Heart Failure in which the device is implanted as a permanent treatment option. The VAD/Heart Transplant Coordinator will also be responsible for the coordination of care for all patients who are pre- or post- heart transplant. The VAD/Heart Transplant Coordinator will work as a member of the Advanced Heart Failure Team, and will be the primary liaison with the transplant cardiologists and cardiac transplant surgeons, other physicians, and health care staff throughout the hospital. A major component of the position is patient and family education, daily rounds, discharge planning, and on-call coverage. The VAD/Heart Transplant Coordinator is also responsible for outreach activity to promote the programs, frequent communication with referring physicians, and professional education. The VAD/Heart Transplant Coordinator functions within the parameters outlined in the LVAD practice guidelines. The Job Standards portion of this job description has incorporated the 17 standards adapted from the ANA Nursing: Scope & Standards of Practice, 2nd Edition (2010). Other duties as assigned.

Requirements

  • Minimum of 3 years of acute care or experience is required.
  • Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required.
  • Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact).
  • Analytical skills necessary in order to prepare and implement effective nursing care and develop solutions to problems concerning patient's mental or physical well-being.
  • Ability to concentrate and pay close attention to detail for up to eighty percent of work time while: planning and performing professional nursing care, resolving patient care problems and dealing with patients, their families, and other professionals involved in the patients care, often during stressful situations.

Nice To Haves

  • Experience relevant to acute care, cardiology, heart failure, VAD care is preferred.
  • Bachelor's degree in Nursing is preferred.
  • Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
