About The Position

TriStar Centennial Medical Center’s premier cardiovascular consultant group is seeking a certified acute care provider for a VAD coordinator role in Nashville, Tennessee. Qualified Candidates: Acute care nurse practitioner or physician assistant certification Tennessee licensure or ability to obtain Two years hospital experience in a cardiac or CV surgical ICU Experience caring for mechanical circulatory support and advanced heart failure patients is preferred Full time, 4 10-hour shifts Shared 24/7 call 1:4 with other coordinators

Requirements

Nice To Haves

Experience caring for mechanical circulatory support and advanced heart failure patients is preferred

Responsibilities

Coordinates and participates in patient evaluation and selection for advanced therapies
Coordinates/leads patient selection and performance improvement meetings
Outpatient clinic visits, clinical management, and care coordination (shared call with 3 other coordinators)
Inpatient discharge planning/care coordination
Inpatient and outpatient education to VAD patients and their caregivers
Oversees/provides education to acute care staff and community resources (EMS, rehab, home health, etc.)
Participates in the development and maintenance of VAD statistical databases and associated registries (INTERMACS)
Equipment and supply management using online inventory tool
Joint Commission certification preparedness
Involvement with community outreach on VAD safety measures and awareness
Liaison between surgeons, heart failure cardiologists, industry partners and patients
