About The Position
The Care Management Coordinator (CMC) plays a crucial role in the healthcare team by serving as a liaison among various disciplines, including Administration, Medical staff, Nursing, and Finance. Under the supervision of the Manager of Case Management and Social Services, the CMC is responsible for ensuring best practices in discharge planning, clinical documentation, and utilization management. The position aims to enhance clinical and cost-efficiency in healthcare delivery while adhering to the organization's mission and values. The CMC is tasked with planning, coordinating, and implementing comprehensive care plans that address the physical, functional, psychological, social, environmental, and financial needs of patients.
