About The Position

Care coordination is an integral part of UroPrime that has had a positive impact on how care is delivered to patients with recurring UTIs. The UroPrime Coordinator role facilitates communication within the interdisciplinary team and the patient to proactively provide individualized, culturally sensitive care that allows patients to have access to quality healthcare through all phases of healthcare experience. What You’ll Be Doing Answering UroPrime patient calls, appropriately answering clinical questions or routing to provider if warranted Provide all clinical documentation in EMR chart related to patient calls, interaction or tasks involved in the care of the UroPrime patient Mail UroPrime kits and/or education/patient material to patients when needed. Use the designated Patient Tracker Database (Case Management in Isalus) to track UroPrime patients from diagnosis to treatment decision and/or treatment. Assist with development and enhancement of processes within the UroPrime program, with the goal of improving clinical experience for referred patients, and better support for the referring physician. Provides clinical support as needed by rooming patients for urology procedures like Fusion prostate biopsies, ensuring a smooth, efficient, and patient-centered experience. Requirements

  • High school diploma
  • Certified Medical Assistant (experience working in healthcare setting preferred)
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine correspondence.
  • Ability to speak effectively before individuals or groups of people.
  • Ability to calculate basic figures and amounts.
  • Ability to apply concepts of basic algebra and geometry.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • To perform this job successfully, an individual should have knowledge of basic computer software.
  • Knowledge of medical terminology to communicate with clinical staff
  • Knowledge of health insurance and prior authorization process
  • Knowledge of common safety hazards and precautions to establish a safe working environment
  • Adapts easily to various environments
