The Crisis Care Coordinator at the Urgent Crisis Center (UCC) plays a vital role in providing immediate behavioral health support to children and youth experiencing crises. This position involves crisis evaluation, de-escalation, and coordination of care within a 24-hour timeframe. The UCC is designed to facilitate fluid movement through the spectrum of services available to families in need, working closely with local hospitals and community agencies. The Crisis Care Coordinator will engage directly with children, adolescents, and their families, assisting them in implementing their crisis plans and connecting them with community resources. In this role, the coordinator will collaborate with Crisis Clinicians to conduct assessments and develop crisis stabilization plans. They will also provide essential support services, including care coordination, referrals, and psychosocial support. The position requires maintaining confidentiality and accurate documentation while ensuring compliance with agency policies. The coordinator will participate in community outreach efforts to educate referral sources about UCC services and may be required to adjust their schedule to meet client needs, including evenings, nights, and weekends. The Crisis Care Coordinator is supervised by the Program Director or their designee and does not have any direct reports. This position is crucial for ensuring that families receive the necessary support during critical times, and it requires a commitment to best practices in crisis intervention and community collaboration.
