Urgent and Primary Care Advanced Practice Provider (NP/PA)
About The Position
Life Medical is a mobile care practice committed to delivering high-quality, on-demand medical services directly to patients in Assisted Living Facilities. We specialize in urgent care visits for individuals who require timely medical attention but face barriers to accessing traditional clinics or emergency departments. Our team is passionate about improving access, reducing unnecessary hospitalizations, and providing compassionate care where it’s needed most. We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our mobile urgent and primary care team. This full-time role involves traveling to Assisted Living Facilities in and around Livingston, NJ to provide urgent medical evaluations and treatments. You’ll work independently with flexible scheduling, supported by a collaborative clinical team.
Requirements
- Licensed NP or PA in the state of New Jersey
- Board-certified in Family Medicine, Adult-Gerontology, or Emergency Medicine
- Minimum of 1 year of clinical experience (urgent care or geriatrics preferred)
- Strong diagnostic and clinical decision-making skills
- Excellent communication and interpersonal abilities
- Valid driver’s license and reliable transportation

Responsibilities
- Conduct urgent or primary care visits in Assisted Living Facilities
- Evaluate and treat acute illnesses, minor injuries, and exacerbations of chronic conditions
- Perform physical exams, diagnostic testing, and procedures as needed
- Prescribe medications and develop treatment plans
- Communicate with facility staff, patients, and families regarding care
- Document visits accurately and promptly in the EMR
- Collaborate with supervising physicians and other healthcare provider
