About The Position

Under Supervision of the Program Manager, the Making The Transition - Peer Navigator (PN) will be responsible for providing peer support and encouragement to members in attending behavioral health appointments and participating in all aspects of the individual’s recovery plan. This position will advocate for program members to receive necessary resources and maintain program members’ progress throughout services.

  • Language: Bilingual in Spanish is required.
  • EDUCATION A Bachelor’s in Psychology, Criminal Justice or related field with 1-3 years of experience in case management is highly preferred; An Associate’s Degree in Psychology, Criminal Justice or related field with 2-4 years of experience working with formerly incarcerated is highly preferred; A High School Diploma or GED Equivalency and 3-5 years of direct experience with AB109 recipients is required in lieu of a degree.

  • Successful completion of Sexual Harassment Prevention Training
  • Successful completion of Workplace Violence Prevention Training
  • Successful completion of HIPPA Training

Responsibilities

  • Adheres to all El Concilio California policies and procedures per the agency employee manual.
  • Greet incoming clients in a professional manner and attend to their inquiries.
  • Conduct initial contact with new referrals no later than two days after receiving referral.
  • Maintain accurate and thorough documentation of program participants contact information.
  • Diligently oversee program calendar arranging conference/video conference calls, virtual meetings on platforms such as Zoom, googlemeets, and Microsoft Teams.
  • Answers incoming calls in a timely manner, avoiding "dropped calls" due to excessive wait time and follows established procedures on how to handle different types of incoming calls.
  • Meet face-to-face with each individual served (and relevant family members) a minimum of one time per week during the initial stage.
  • Use assessment outcomes to inform and update interventions described within treatment plans.
  • Offer groups that focus on increasing opportunities for those served to improve and/or develop their social support network.
  • Develop and deliver presentations, workshops, and campaigns on behavioral health awareness, stigma reduction, and suicide prevention.
  • Facilitate peer-led or lived-experience discussions when appropriate, fostering open conversations about mental wellness.
  • Communicate regularly with members to gather necessary information, answer general questions, and keep them informed about case progress.
  • Facilitate communication and collaboration among service providers, family members, and other supports.
  • Prepare and file orientations, intakes, and assessments
  • Develop educational material in the form of Booklets, flyers, and PowerPoint presentations to present to program members.
  • Introduced members to other services and support systems available in the community such as Wellness Centers, Peer Navigation, and Housing Empowerment Programming.
  • Initiate referrals and advocate for program participants to receive adequate information to outside resources.
  • Assist program participants with the completion of any formal documentation that will provide access to resources, such as housing, education, transportation, clothing, food, employment, etc.
  • Aim for a collective goal of serving up to 100 AB109 clients based upon an average client service period of six (6) months.
  • Perform outreaching at local tabling events to recruit program participants.
  • Participate in interdisciplinary team meetings, trainings, and case consultations.
  • Maintain awareness of all department caseload and statuses.
  • Maintain professional relationship with all program participants and El Concilio California personnel.
  • Required to work on evenings and weekends for El Concilio California special events and necessary training.
  • Other duties as assigned by immediate supervisor and administration.
