About The Position

The division is actively recruiting for an Advanced Practice Provider interested in working full-time in the care of patients with diabetes who are preparing for surgery. The successful applicant will coordinate with multiple surgical divisions to identify patients with diabetes who need surgery. The APP will perform an assessment and provide an individualized pre-habilitation framework so patients can train for surgery. The goal is to maximize patient health before a surgical procedure and teach patients life-long skills to help them better manage their health after surgery. The provider will need to coordinate care with surgical teams, endocrinology support, and anesthesia pre-operative teams. The provider will also work closely with a data abstractor to assist with obtaining and documenting important information to track patient outcomes.

Requirements

  • Completion of an accredited Physician Assistant program and currently licensed or eligible for licensure as a Physician Assistant
  • Completion of an accredited Nurse Practitioner Program and currently licensed or eligible for licensure as an APRN
  • Minimum of 1 year of experience as a PA, FNP or ANP

  • Clinical Nurse Specialist in Advanced Diabetes Management
  • Experience managing patients with diabetes and/or advanced diabetes management training
  • CDCES qualification

Responsibilities

  • Takes comprehensive history and performs physical examinations
  • Evaluates and treats on the basis of history, physical examination, radiological, laboratory, and other diagnostic test results pursuant to the practice agreement or scope of practice guideline
  • Initiates referrals to other health care providers, and/or consults with the attending physician or the collaborating physician
  • Documents and bills for direct care provided
  • Utilizes current research and evidence-based decision-making in all clinical practice
  • Performs and participates in quality/performance improvement activities and clinical research
  • Participates in and supports accreditation, compliance, and regulatory activities of the organization
  • Demonstrates responsibility for professional practice through active participation in professional organizations and continuing education
  • Potential to precept students at MUSC enrolled as an advanced practice provider student; and facilitates the learning of other new team members
  • Coordinate with multiple surgical divisions to identify patients with diabetes who need surgery
  • Perform an assessment of pre-operative patients and provide an individualized pre-habilitation framework
  • Coordinate care with surgical teams, endocrinology support, and anesthesia pre-operative teams
  • Work closely with a data abstractor to assist with obtaining and documenting important information to track patient outcomes
  • Work closely with South Carolina Surgical Quality Collaborative and Diabetes Free South Carolina leadership as the MUSC point of contact for the prehabilitation program
