The LPN Unit Manager at The Center at Cordera plays a pivotal leadership role, overseeing and coordinating the nursing care on a designated unit. This position is responsible for ensuring the delivery of high-quality, resident-centered care in accordance with professional standards, facility policies, and regulatory requirements. The LPN Unit Manager will provide clinical supervision to LPNs and Certified Nursing Assistants (CNAs), promote a collaborative team environment, and act as a key liaison between residents, families, physicians, and the interdisciplinary team.

Requirements

  • Current and unencumbered LPN license in the State of Colorado.
  • Graduate of an accredited School of Practical Nursing.
  • Minimum of 2 years of clinical experience as an LPN, with at least 1 year in a skilled nursing or rehabilitation setting preferred.
  • Current CPR certification.
  • Strong clinical assessment, critical thinking, and problem-solving skills.
  • Excellent communication, interpersonal, and leadership abilities.
  • Proficiency in electronic health records (EHR) systems.
  • Ability to work effectively in a fast-paced, team-oriented environment.
  • Dedication to providing compassionate, resident-centered care.

Nice To Haves

  • Previous supervisory or charge nurse experience highly desirable.

Responsibilities

  • Clinical Leadership & Supervision: Provide direct supervision, guidance, and support to LPNs and CNAs on the unit.
  • Monitor the delivery of nursing care to ensure adherence to established care plans, physician orders, and facility protocols.
  • Conduct regular rounds to assess resident conditions, anticipate needs, and ensure a safe and therapeutic environment.
  • Assist with medication administration, treatments, and direct patient care as needed.
  • Respond promptly and effectively to resident emergencies and changes in condition.
  • Care Coordination & Planning: Collaborate with the interdisciplinary team (RNs, therapists, social workers, physicians) to develop, implement, and evaluate individualized care plans.
  • Participate in resident admissions, transfers, and discharges, ensuring smooth transitions and accurate documentation.
  • Communicate effectively with residents and their families regarding care plans, progress, and any concerns.
  • Staff Management & Development: Assist with scheduling, assignments, and performance monitoring of unit staff.
  • Provide ongoing coaching, feedback, and mentorship to promote professional growth and maintain high standards of care.
  • Foster a positive, respectful, and collaborative work environment.
  • Participate in staff education and in-service training as required.
  • Documentation & Compliance: Ensure accurate, thorough, and timely documentation of all nursing care, observations, and interventions in resident records.
  • Maintain compliance with all federal, state, and local regulations, as well as facility policies and procedures.
  • Participate in quality assurance and performance improvement initiatives.
  • Communication & Problem Solving: Serve as a primary point of contact for resident and family concerns on the unit, addressing issues promptly and professionally.
  • Report significant changes in resident condition or critical incidents to the appropriate nursing leadership and medical staff.
