About The Position

The Unit Manager - LPN is responsible for overseeing the day-to-day operations of a specific nursing unit in a healthcare, long-term care, or assisted living facility. This position involves supervising nursing staff, coordinating patient care, ensuring compliance with regulations, and maintaining high-quality resident care. The Unit Manager - LPN collaborates with other healthcare professionals to manage clinical services, create care plans, and address patient needs, while also handling administrative duties related to staffing, documentation, and compliance.

Requirements

  • Education: Graduate of an accredited LPN program.
  • Licensure: Current and active Licensed Practical Nurse (LPN) license.
  • Experience: Minimum of 2-3 years of nursing experience, preferably in a long-term care or assisted living setting, with at least 1 year in a supervisory or leadership role.
  • Skills: Strong leadership and team management abilities.
  • Excellent clinical skills, with the ability to assess, plan, and implement patient care.
  • Strong organizational and time-management skills, with the ability to prioritize tasks in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Proficiency in electronic health records (EHR) systems and documentation.

Responsibilities

  • Unit Leadership and Supervision:
  • Supervise and manage nursing staff within the assigned unit, including Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and other support staff.
  • Provide leadership, guidance, and support to staff, ensuring high-quality patient care and adherence to facility protocols.
  • Conduct staff evaluations, offer performance feedback, and assist with professional development and training.
  • Create and manage staff schedules, ensuring adequate staffing levels for each shift.
  • Patient Care Management:
  • Oversee the delivery of patient care within the unit, ensuring that residents receive appropriate, individualized care based on their needs and care plans.
  • Collaborate with physicians, Registered Nurses (RNs), therapists, and other healthcare professionals to develop and implement care plans.
  • Monitor residents' conditions, address any concerns, and ensure timely communication with families and medical teams regarding changes in health status.
  • Provide direct nursing care as needed, including medication administration, wound care, and other treatments.
  • Compliance and Documentation:
  • Ensure that all nursing staff comply with federal, state, and local regulations, as well as facility policies, regarding patient care and safety.
  • Maintain accurate and timely documentation in the electronic health record (EHR) system, ensuring all patient care activities, assessments, and treatments are properly recorded.
  • Conduct regular audits of clinical documentation to ensure completeness, accuracy, and compliance with regulatory standards.
  • Participate in facility inspections, surveys, and audits, addressing any areas of non-compliance and implementing corrective actions.
  • Quality Assurance and Improvement:
  • Implement and monitor quality improvement initiatives within the unit, ensuring that care standards meet or exceed regulatory and facility guidelines.
  • Participate in care conferences, team meetings, and quality improvement committees to review resident outcomes and address any care-related concerns.
  • Ensure that the unit adheres to infection control protocols and other safety measures to protect residents, staff, and visitors.
  • Resident and Family Relations:
  • Serve as a liaison between residents, families, and the healthcare team, providing updates on care plans, treatments, and health status.
  • Address any concerns or complaints from residents and families in a professional and compassionate manner, escalating issues to senior management as needed.
  • Ensure that residents' dignity, privacy, and personal preferences are respected at all times.
  • Collaboration and Communication:
  • Collaborate with other department heads, including dietary, social services, and therapy, to ensure comprehensive and coordinated care for residents.
  • Facilitate communication between nursing staff and other members of the interdisciplinary care team to ensure effective care planning and delivery.
  • Participate in care planning meetings, discharge planning, and other interdisciplinary team meetings to support optimal resident care.
