About The Position

The Unit Clinical Care Coordinator - Emergency Department (Unit C3-ED) is responsible for managing emergency department admission flow, patient progression, care coordination, and discharge planning to enhance system efficiency. This role serves as a liaison between the Emergency Department and the Hub, ensuring that patient care needs are met and treatment plans are executed in a timely manner. The coordinator leads collaboration among healthcare team members to improve patient throughput and achieve effective patient-focused outcomes.

Requirements

  • Minimum of three (3) years recent acute care nursing experience.
  • Ability to work independently and manage multiple priorities effectively.
  • Ability to maintain a calm demeanor during crisis management situations.
  • Ability to quickly assimilate information and make sound decisions.
  • Strong written and verbal communication skills.

Nice To Haves

  • Bachelor of Science in Nursing is preferred.
  • Emergency Department acute care nursing or case management experience is preferred.
  • One (1) year supervisory experience is strongly preferred.

Responsibilities

  • Assess ED patients' clinical presentation to identify admission appropriateness and communicate bed needs to facilitate admission flow.
  • Coordinate patient admission and care activities with the Hub to ensure timely patient placement and progression.
  • Identify patients with repeated ED visits or unmet social/community needs and partner with relevant services to determine admission appropriateness.
  • Lead clinical care team huddles to identify patient progression and discharge barriers, assigning tasks for resolution.
  • Develop and arrange comprehensive discharge plans addressing patient needs and readmission risk factors.
  • Communicate the plan of care and discharge plan to patients and their families in coordination with the care team.
  • Manage all aspects of admission, patient progression, and discharge in the hospital's bed management system.
  • Ensure timely compliance with preprocedural requirements for diagnostics and procedures.
  • Identify and resolve barriers to patient flow, escalating unresolved issues to senior leadership.
  • Coordinate with Utilization Review Nurse to align payor and status requirements with patient clinical progression.
