About The Position

The Children's Home of Poughkeepsie has been caring for children throughout the Hudson Valley for over 175 years! We strive to empower children and families to achieve optimal health and well being. We are currently seeking a UC Case Manager to join our UC LTFC Program. Position Summary: The Case Managers is responsible for the delivery of case Management services to unaccompanied Children, UC, In care. The Case manager is responsible for all case management activities which includes: contacting/locating sponsors, and facilitation of the reunification process of UC-sponsor, including travel to various locations in the US. He/She contributes to the treatment plan development, completion of UC portal documentation, and maintenance of UC paper and electronic records and may attend immigration court with UC. Which also includes conducting a safe and timely release, verifying relationships, and ensuring the safety of the household prior to discharge.

Requirements

  • Bachelor's degree in psychology, Human Services, or other related field.
  • Related Child Welfare and/or case management experience.
  • New York State Driver's License in good standing as determined by the Agency.

Responsibilities

  • Responsible for working with the child and the child's family/sponsor. This includes locating family members, assessing potential sponsors to unify the child with their families/sponsor.
  • Is sensitive to and supportive of the children’s cultural and ethnic heritage and values.
  • Assess the needs of each unaccompanied child in care, while developing an Individual Service Plan, and facilitate a safe timely release/discharge, in collaboration with the Clinician.
  • Plans with the Clinician for conferences, phone calls, and family visits which are to be coordinated with child care according to team plans.
  • Responsible for maintaining case files on his/her caseload and document the provision of services in each UAC’s case file while maintaining ORR/UAC database compliance.
  • Responsible for completing initial intake, assessments, case reviews, and weekly case notes to meet programs and ORR deadlines.
  • Collaborates with the Clinician for evaluations and team recommendations/ safety planning.
  • Collaborates effectively with both internal teams and external entities.
  • Participate in multidisciplinary team conferences and meetings.
  • Coordinates the discharge or transfer of each child including post-release services.
  • Report and discuss cases with third party reviewers, clinical staff, and Federal Field Specialist.
  • Responsible for continued professional development through attendance at in‑service seminars, participation in conferences and workshops and/or continued study at higher education institutions.
