About The Position

This position involves working with the Trinsic department, an accountable care organization focused on Value Based Care (VBC) populations. The role is primarily responsible for assessing and documenting socio-economic factors affecting patients and their families, planning interventions to navigate the healthcare system, and providing clinical support related to VBC programs. The position is fully remote and operates on a day shift schedule, Monday to Friday, with varying hours.

Requirements

• Experience as a Medical Assistant, in Care Management, or in Primary Care Practices.
• Knowledge of Social Determinants of Health and Value Based Care.
• Familiarity with Quality Metrics and Public Speaking.

Responsibilities

• Assess and document patients' socio-economic reviews and screenings to support care coordination.
• Plan interventions to help patients navigate the healthcare system and reduce barriers to care.
• Provide clinical support and coordination related to value-based care programs.
• Participate in huddles with primary care practices to assist with scorecard initiatives and program metrics.
• Consult with care teams to provide care coordination for preventative care and utilization mitigation.
• Identify appropriate resources for patients, including transportation, housing, healthcare, and social services.
• Support providers and practice teams on office-based care delivery interventions.
