The Triage RN is responsible for ensuring the smooth delivery of Home Health services by coordinating communication, services, and orders between patients, providers, and clinicians to meet patient/family needs. The Triage RN functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Alaska state Board of Nursing and based on the nursing process model. Shift, schedule and caseload of shifts and caseload may vary according to position assignment.

  • Perform triage activities such as receiving incoming phone calls from home health patient provider, providers' staff, patients and families, assessing patient needs, providing immediate troubleshooting/problem-solving over the phone.
  • Provide nursing advice and triage using sound nursing assessment skills and following approved protocols.
  • Document all patient encounters in the electronic medical record and other platforms as required, accurately and timely.
  • Contact providers to obtain verbal orders and process appropriately for treatment, labs, wound care, extensions of service, addition/removal of services, and more.
  • Arrange for in-person or virtual nursing visits as indicated and provide follow up communication to other staff regarding situation and needs.
  • Maintain relationships that promote optimal patient outcomes.
  • Communicate in an efficient and positive manner that fosters productive relationships and maintains high quality care.
  • Collaborate with community and bedside healthcare providers to ease transitions in care and set an individualized care plans that will enable the patient to be successful in managing their acute or chronic conditions.
  • Attend training opportunities and maintain technological competence.
  • Follow established workflows, policies, and procedures.
  • Contribute to overall team function through effective time management and communication; utilize critical clinical thinking and problem-solving skills to address team issues.
  • Participate in Continuous Quality Improvement activities, identify related issues and implement recommended changes as appropriate.
  • Associate's Degree Nursing Or Bachelor's Degree Nursing
  • Graduate of a school of nursing.
  • 2 years Nursing experience.
  • 1 year Recent clinical experience in any of the following suggested areas; home health, tele health, medical-surgical, critical care, emergency, pediatric or maternal-child health nursing
  • 1 year Home Health RN within the past 3 years.
