Job Summary The Registered Nurse – Triage & Nurse Navigator plays a critical role in patient care by performing evidence-based telephone triage, guiding patients through the healthcare system, and ensuring continuity of care. This position combines clinical expertise with patient advocacy to deliver safe, timely, and compassionate care.

Requirements

  • Extensive experience working with patients in critical conditions.
  • Sound knowledge of the healthcare system and where to find reliable information.
  • Excellent verbal and written communication skills.

  • Sensitivity, discretion, and respect.
  • Communicate professionally and maintain confidentiality in compliance with HIPAA regulations.
  • Graduate of an accredited NLN/CCNE nursing program required
  • Current RN licensure in state or compact state required.
  • Basic Life Support (BLS) certification required.
  • Minimum 5 years of RN experience required
  • 3 years of triage experience required
  • New Hires are required to pass pre-employment background check and drug testing (effective 11/1/2022)
  • Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire.

Nice To Haves

  • BSN preferred.
  • Outpatient, or nurse navigator experience preferred.

Responsibilities

  • Triages patient’s healthcare needs and makes decisions regarding specific concerns and findings, following appropriate telephone triage protocols.
  • Updates and maintains the patient’s medical record.
  • Interfaces with multidisciplinary team members to provide optimum care.
  • Utilizes effective communication and interpersonal skills to enhance and encourage collaborative staff and community relationships.
  • Monitors clinic schedule and patient care activities.
  • Utilizes the Nursing Process (subjective and objective data collection, formulation of a plan and diagnosis, implementation of the plan of care, and evaluation of the patient’s response to intervention) and current evidence- based knowledge to provide comprehensive care to a designed patient population.
  • Contributes to the recruitment and retention of talented staff through positive role-modeling, fostering peer relationships, and participating in precepting medical students and creating an environment for quality practice.
  • Assumes responsibility for their role in achieving/maintaining financial health of the organization by utilizing all resources effectively and efficiently.
  • Identifies and recommends service and quality improvement initiatives.
  • Participates in continuing education and quality improvement activities.
  • Supports membership in professional activities.
  • Utilizes knowledge of the relevant Nurse Practice Acts, compliance, regulation, legislation, certification, and department policies.
  • Follows Patient Care Policies, the Family Medicine Center policies, organizational policies, and regulatory requirements in patient care.
  • Guide patients through the healthcare process from initial contact to treatment and follow-up.
  • Educate patients on diagnoses, treatment plans, and potential side effects in clear, accessible language.
  • Provide emotional support and connect patients with resources, including social work or counseling services.
  • Assist patients in overcoming barriers such as transportation or financial challenges.
  • Schedule timely appointments for tests, procedures, and follow-up care.
  • Collaborate with physicians, PEC staff, and Population Health teams to ensure continuity of care.
  • Communicate professionally and maintain confidentiality in compliance with HIPAA regulations.
