About The Position

The Clinic Certified Registered Nurse Triage is responsible for assessing patient symptoms and providing medical advice based on clinical protocols and guidelines. Demonstrates knowledge of the nursing process as a standard, the RN collaborates with providers, interdisciplinary teams, and patients to assess, plan, and implement and evaluate safe and effective care. This role requires strong clinical and communication skills to ensure the provision of quality patient care. All staff is considered to be an employee of St. Peter's Health Medical Group first. Because of this, there is an expectation that they may float to other SPHMG areas when not needed in assigned area.

Requirements

  • Clinical medical, surgical, pediatric, or OB experience of 5+ years preferred.
  • Graduate of accredited professional school of nursing.
  • Current Montana License as a Registered Nurse required.
  • Current Basic Life Support (BLS) required.
  • Strong clinical assessment skills and knowledge of medical protocols.
  • Ability to achieve cognitive, organization and emotional maturity to deal effectively with multiple tasks, stresses, deadlines, difficult situations and/or customers.
  • Ability to conduct self professionally, including but not limited to: interpersonal/communication skills necessary for effective, non-judgmental, and empathetic patient care and customer relations.
  • Open to feedback, and open to a changing environment, which requires flexibility in scheduling and department assignments.
  • Strong ability to communicate effectively both verbally and in writing.

Nice To Haves

  • Triage experience preferred (walk-in or telephone).
  • Proven ability to work independently in a remote environment, demonstrating strong time management, communication, and accountability to complete tasks and meet deadlines with minimal supervision preferred.

Responsibilities

  • Patient Education & Communication Educate patients and families on disease management, medications, treatment plans, and preventative care, including how to manage at home
  • Serve as a patient advocate and assist patients in navigation along the care continuum with the goal of facilitating effective quality care and timeliness, resulting in improved patient satisfaction and outcomes; acts as a liaison between patients, providers, and other healthcare team members.
  • Adapt teaching methods to meet diverse learning needs
  • Triage/Assess patient concerns/symptoms over the phone, in person, or electronic communication
  • Provide timely medical advice and guidance based on clinical assessments, protocols, and guidelines
  • Triaging referrals using applicable guidelines and protocols, promoting timely scheduling and processing of referrals to optimize patient care and workflow efficiency
  • Manage the primary care in-basket with emphasis on high-priority messages, providing timely assessment, initial clinical recommendations within RN scope, and escalation to providers as needed.
  • Determine the urgency of patient conditions and recommend appropriate care levels
  • Maintain accurate and timely electronic health records (EHR)
  • Document all nursing assessments, interventions, and communications within the EHR
  • Utilize EHR, RightFax, and other healthcare technologies effectively
  • Answer phones real-time, return patient messages, and coordinate communication between the care team and patients.
  • Assist with management of clinical staff in-basket messages
  • Must be able to use critical thinking and leadership skills to prioritize needs of clinic, allowing for most efficient flow, and optimal patient experience
  • Utilize all resources and guidelines at his/her disposal to effectively assess, prioritize, advise, schedule appointments, or refer calls when necessary to the appropriate personnel
  • Participate in continuous quality improvement initiatives
  • Identify opportunities to enhance patient care and clinic operations
  • Works collaboratively with other members of the healthcare team, co-workers, physicians, and the community emphasizing a teamwork environment
  • Other duties as assigned
  • Required to accommodate for and attend 80% of all scheduled meetings.
