About The Position
The Triage RN Coach will operate in a telephonic virtual care center as part of the Care Connections Team, collaborating with healthcare professionals to provide high-quality care across the healthcare continuum. This role focuses on acute patient triage, ensuring safety and best practices while assisting patients in navigating the healthcare system. The Triage RN Coach will assess patient needs over the phone, provide relevant health information, and promote patient engagement to reduce inappropriate care venues and hospitalizations.
