About The Position

Triage Nurse (RN) - Monday through Friday, no weekends or holidays location: in office SUMMARY OF JOB RESPONSIBILITIES The Triage Nurse prioritizes and responds to calls received from hospice and palliative care patients/families, in a timely manner. Incorporates nursing skills and appropriate questions to accurately evaluate problems/needs and provide education to the patient/caregiver in accordance with the plan of care. Provides emotional support appropriately during the telephone conversations to patients/caregivers. Determines appropriate discipline to respond to call and/or visit patient/family. Conducts follow up calls and/or visits as problems/needs arise or as requested by team members or supervisor. ESSENTIAL FUNCTIONS AND DUTIES (other duties may be assigned) Maintains the confidentiality of patient and employee information at all times, both at work and off duty, and after the termination of employment, according to the agencies Notice of Privacy Practices and HIPAA regulations. Is knowledgeable and functional in computer systems including Microsoft Applications, Teams and e-mail system as required for the execution of the job. Is knowledgeable of hospice concept and philosophy. Proficient in telephone techniques including customer service, phone etiquette, and handling of calls. Demonstrated proficiency working within a fast-paced team environment and apply critical thinking and problem solving skills independently. Obtains and assesses clinical information regarding patients’ condition and responds accordingly and timely to accepted standards of nursing practice. Communicates accurate information reflecting knowledge of agency services, regulations and policies and procedures. Follows up with concerns and brings unresolved issues to Clinical Manager. Works cooperatively on projects and tasks with the Clinical Manager in maintaining compliance with Medicare/Medicaid, State and Federal Regulations. Provide in field and video patient care/visits as needed to assure effective patient symptom management. Participate in on call schedule as needed including weekend rotation. Provide support to Clinical Manager by assisting with a variety of clerical tasks. Reviews and follows up from on call report. Accepts orders from providers, confirms in writing according to agency policy and routes information to appropriate caregiver/team including notifying staff in field as needed. Fill medication orders through e-prescribing, order DME. Documents all interactions with patients, families, contacts, and vendors in the EMR. Performs other duties as assigned by Clinical Manager/Educator.

Requirements

  • Registered Nurse with degree from an accredited nursing school and holds a current RN license in the State of Michigan.
  • Possesses and maintains a current CPR certification.
  • Maintains a valid Michigan driver’s license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Nice To Haves

  • Minimum of three years of nursing practice, with at least one year in hospice or home care is preferred.
