The position involves performing initial consultations with patients, taking vital signs, and assessing symptoms and health-related complaints. The role serves as a resource to staff by suggesting solutions to patient care crisis problems and complaints. The individual will coordinate patient care activities, conduct clinical interviews, educate patients and families, perform telephone triage, communicate physician's instructions, assist in medical record completion, and maintain departmental policies and procedures.

  • Coordinate patient care activities for the clinics by communicating with patients, pharmacies, hospitals, and other related entities.
  • Conduct clinical interviews with patients to assess a variety of patient needs.
  • Educate patients and families about diagnostic procedures, medications, nutrition, and maintenance of health and wellness.
  • Perform patient telephone triage, assessing needs and coordinating with medical staff.
  • Communicate and interpret physician's instructions to patients.
  • Suggest solutions to patient care crisis problems and complaints.
  • Assist physicians in completing medical records concerning medical history, diagnosis, prognosis, and current treatment plans.
  • Maintain required documentation, records, charts, and reports.
  • Assist in developing and maintaining departmental quality plan.
  • Complete other job-related duties and projects as assigned.
  • Successful completion of a nursing program from an accredited school of nursing.
  • Current license to practice as a LPN in Indiana required; a licensed Registered Nurse is preferred.
  • CPR certification.
  • Two years of professional nursing experience.
