About The Position

POSITION SUMMARY: This position is responsible for providing patient care in a compassionate, professional, ethical, and knowledgeable manner. This includes, but is not limited to: assessment, planning, implementation and evaluation of the patients' status, plan of care and goal achievement. This position is responsible for triaging patients to determine proper next steps of the patient's care. Additionally, this position is responsible for providing administrative support by performing and completing various departmental tasks. ESSENTIAL JOB DUTIES: Follows triage protocols, policies, and procedures, including answering, documenting, and appropriately routing phone notes. Processes outpatient and pre-operative lab results; enters pre-hydration orders when indicated and routes to Advanced Practice Providers (APPs) for review and signature as needed. Completes all daily departmental tasks, including managing phone notes, voicemails, system flags, medication refills, and cardiac/dental clearance requests. Performs TCM (Transitional Care Management) clinical intake, including reviewing discharge documentation, reconciling medications, assessing patient status, and ensuring all required elements are documented for billing compliance. Provides patient education and coordinates care for patients enrolled in the VIP program, including reinforcing care plans, monitoring engagement, and collaborating with the care team. Assesses and evaluates patient responses to treatments, medications, and services provided. Demonstrates knowledge of nursing theory, principles, and clinical practice to deliver comprehensive, patient-centered care. Communicates effectively with patients, families, and staff using clear and professional verbal and written communication. Actively seeks educational opportunities to expand knowledge across age groups, disease states, and best practices. Adheres to all organizational policies and procedures while upholding the standards and expectations of Cardiac Solutions. Participates in interdisciplinary team meetings and other relevant organizational meetings. Shares clinical knowledge and experience to support team learning and development. Assists with the orientation, training, and coaching of new and existing staff. Performs other duties as assigned, consistent with organizational needs and within scope of licensure.

Requirements

  • Knowledge of Microsoft Office products.
  • Knowledge of HIPAA regulations.
  • Skill with the nursing process and its application, including the assessment, planning, implementation, and evaluation of nursing care.
  • Ability to maintain confidentiality and professionalism.
  • Ability to multi-task.
  • Ability to work in a fast paced-environment and work well under pressure.
  • Ability to work effectively with others.
  • Ability to travel to other sites as needed.
  • License Practical Nurse from an accredited College of Nursing.
  • Valid Arizona Nursing License.
  • Minimum of two (2) years of successful nursing practice.
  • Experience using an Electronic Medical Record (EMR).
  • Excellent verbal and written communication skills.
