Triage Case Manager (2nd Shift)
About The Position
Renaissance Social Services, Inc. (Renaissance) is a leader in Chicago’s response to homelessness, preventing and ending homelessness for vulnerable Chicagoans by providing a range of proven solutions including prevention, outreach, rapid re-housing, permanent supportive housing, behavioral health care, and clinical case management. The Triage Bridge Housing Program offers temporary housing and assessment for unhoused individuals coming from the street or shelters, engaged through the agency’s Homeless Outreach Program or referrals from Chicago Continuum of Care partner agencies. This will include individuals with co-occurring serious mental illnesses, substance use disorders, and other factors. The Skilled Case Manager provides intensive Case Management and supportive services to program participants in order to prepare them for placement into permanent housing, including conducting a comprehensive assessment to ensure program participants are connected to the most appropriate housing program. This position collaborates closely with Chicago’s Coordinated Entry System to receive and connect program participants.
Requirements
- Must meet the State of Illinois criteria for a Mental Health Professional (MHP) via one of the following pathways: Bachelor’s degree in a human services field (Social Work, Psychology, Counseling, etc.); Bachelor’s degree in an unrelated field plus two (2) years of documented clinical experience in a mental health setting; or High School Diploma or GED plus five (5) years of documented, supervised clinical experience in mental health or human services.
- A minimum of 3–5 years of clinical experience, with a strong preference for candidates with a background in case management or direct community-based service.
- Proven experience working with individuals experiencing chronic mental illness and substance use disorders, with a firm commitment to Harm Reduction principles.
- Ability to relate appropriately and effectively with individuals and families, as well as with staff and personnel from other community agencies.
- Strong computer literacy, including the ability to utilize Microsoft Office Suite and Electronic Health Record (EHR) systems for clinical documentation.
Nice To Haves
- Active Certified Case Manager (CCM), CRSS (Certified Recovery Support Specialist), or CPRP (Certified Psychiatric Rehabilitation Professional) designation.
- Experience in triage, crisis intervention, and rapid assessment within a housing or clinical setting.
- Skill in organizing and documenting clinical progress notes and comprehensive service plans in accordance with HIPAA.
- Ability to work effectively as a member of an interdisciplinary team to ensure continuity of care.
Responsibilities
- Identifying program participants’ needs, including barriers to receiving appropriate services and assisting with effective problem solving to address issues.
- Providing information, linkage, referral and advocacy for program participants with agencies in the community, including crises intervention services when needed.
- Working with the Coordinated Entry System to receive program participants from homelessness and transition to permanent housing.
- Maintaining the confidentiality of client information in any written, oral, or electronic form including, but not limited to, health information, HIV/AIDS status, and disability information. Releasing such information only as needed and with the proper release from the client unless mandated by law.
- Ensure program participants are eating and have access to the food they need.
- Reporting all incidents of abuse, and/or neglect as required by state law.
- Maintaining accurate and up-to-date records using HMIS and paper files on all program participants receiving services, including maintaining records necessary to fulfill grant requirements and for any third-party payers.
- Coordinating the provision of appropriate services from other providers from the community at buildings in which program participants are living.
- Working cooperatively with entire team to ensure residential stability of program participants.
- Identifying and developing training opportunities for program participants.
- Completing required documentation for Coordinated Entry, HMIS and compliance in a timely manner.
