Requirements
- Current California LVN license in good standing
- Experience in wound care/treatments in a skilled nursing or post-acute setting preferred
- Strong assessment and documentation skills
- Team-oriented, reliable, and compassionate bedside manner
- Ability to work efficiently while maintaining quality resident care
Responsibilities
- Provide skilled wound and skin care treatments per physician orders and facility protocols
- Perform wound assessments and accurately document progress and changes
- Communicate treatment updates with the care team, providers, and families as needed
- Assist with care planning and ensure proper implementation of wound prevention strategies
- Support infection control standards and monitor for signs/symptoms of infection
- Collaborate with nursing leadership and interdisciplinary teams to promote best outcomes
- Ensure accurate and timely treatment documentation in the medical record
