About The Position

Our Treatment Foster Care Program in Towson, MD has an immediate opening for a Treatment Coordinator (TC). The TC partners with families and community agencies to ensure the therapeutic needs of youth and families are individualized, child-centered, family focused, culturally competent, strengths based, and goal driven. This position requires you to obtain and maintain the Licensed Bachelors Social Work licensure. If you do not have this license, we will help you prepare to obtain it!

Requirements

Clearances. State Police/FBI clearance per state regulation; Child and Adult Abuse Registry clearance
Valid drivers license and current auto insurance.
Reliable transportation for frequent travel to various community locations for client services and home visits.

Responsibilities

Assess child needs, develops measurable objective treatment plan goals, provides on-going advocacy and support, and monitor progress to assure development and implementation of the services provided.
Works in collaboration with other service providers to develop and implement intervention procedures in the homes working toward achievement of the permanency plan.
Maintains regular contact and open lines of communication with youth's treatment team and any other significant community persons.
Assist and support in training activities for Treatment Foster Care (TFC) parents.
Completes assessments and ensure all documentation is uploaded into the EHR system within a timely manner.
Directly supervise all activities of treatment parents related to the treatment and care of TFC youth through regular home visits and phone calls.
Ensure adherence to TFC model fidelity through completion of TFC trainings, parent support and skill forms and parent evaluations.
Ensures completion of and collect TFC parent documentation, including LODES, Medication Logs, Respite logs, copies of medical information and any other required documentation.
Complete a formal TFC Parent Evaluation, including creation of a professional development plan for each family.
Assist in the arrangement of respite services.
Maintain documentation of in-home and other ongoing training activities.
Prepare written summaries, Individual Service, Permanency, Safety,, and Discharge Plans.
Ensure completion and collection of TFC parent documentation for required forms.
