Requirements
- MN PT license in good standing
- Solid documentation and communication skills
- Driver’s license, vehicle, and current insurance
- A desire to empower clients within their home environment
Responsibilities
- Perform evaluations and develop home -based therapy plans
- Collaborate with families, nurses, and care teams
- Document care accurately and adjust plans as needed
- Help clients achieve personal milestones through mobility and strength coaching
