Join our mission-driven team supporting PACE (Program of All-Inclusive Care for the Elderly) participants in Fort Gratiot, MI. This part-time role is perfect for pharmacists passionate about geriatric care and interdisciplinary, patient-centered practice.
Requirements
- PharmD from an accredited program
- Active Pharmacist License & Controlled Substance License
- 2+ years experience in geriatrics, long-term care, or related setting
- Willingness to travel to Michigan cities of Alpena, Ft. Gratiot, and Mt. Pleasant
Responsibilities
- Review and manage medication regimens for older adults with complex needs
- Conduct medication reconciliations and chart reviews
- Identify, resolve, and prevent medication-related problems
- Participate in interdisciplinary team (IDT) meetings and care planning
- Ensure compliance with CMS, state pharmacy laws, and PACE standards
- Provide education to participants, caregivers, and healthcare team members
