About The Position

"Navitas Healthcare, LLC" is seeking an experienced LCSW to support behavioral health patients from admission through discharge. The role includes clinical assessments, treatment planning, therapeutic interventions, crisis support, and coordination of safe, effective care transitions. Works closely with psychiatrists, the interdisciplinary team, families, and community partners to ensure appropriate services and compliance with all regulatory standards.

Requirements

• Active MA Licensed Clinical Social Worker (LCSW).
• CPI training and BLS required.
• Strong communication, assessment, and clinical decision-making skills.

Responsibilities

• Complete behavioral health assessments and develop treatment plans.
• Lead discharge planning and arrange aftercare services.
• Provide group, individual, and family therapy.
• Evaluate psychosocial, medical, and home-care needs; connect patients to resources.
• Support crisis prevention/intervention and collaborate with community agencies.
• Obtain insurance authorizations and ensure CMS/DPH/DMH compliance.
• Provide coverage for inpatient and partial hospital programs.
• Deliver high-quality care and participate in outreach as needed.
