About The Position

Heartline Staffing, a Strategic Talent Partner with Purple Cow Recruiting, is seeking an experienced Registered Nurse Case Manager for a travel assignment supporting an acute care hospital in Mission Hills, California. This role focuses on inpatient care coordination and discharge planning for medically complex patients in a hospital setting.

ASSIGNMENT DETAILS
This is a travel Registered Nurse Case Manager role in an acute care hospital environment. The nurse will support care coordination, utilization review, and discharge planning for adolescents, adults, and geriatric patients. The role includes close collaboration with interdisciplinary teams including social services and transportation.

SCHEDULE
Day shift
5x8 schedule
Hours are 8:00 AM to 4:30 PM
Weekend rotation required to meet department needs

Acute care hospital setting
Average patient ratio is 1:20
Epic electronic medical record system
Scrubs or business attire permitted

COMPENSATION
Gross weekly pay TBD
W2 hourly rate TBD

Requirements

• Minimum of 2 years of acute care Hospital Case Manager experience performing Care Coordination and Discharge Planning. Hospital experience is required.
• Candidates with behavioral health only experience should not be screened and should be redirected.
• Travel experience is required. First time travelers are accepted and will be screened by clinical.
• Active California RN license is required. Pending licenses are accepted but must be active prior to the start date.
• Experience utilizing InterQual Criteria is required.
• Epic charting experience is required.
• Ability to independently arrange DME, SNF placement, and ambulance transport is required.
• Weekend rotation is required.
• Requested time off is limited to no more than 7 days.
