About The Position

We are seeking an experienced Travel RN Case Manager to coordinate patient care from admission through discharge. This role focuses on ensuring appropriate level of care, reducing avoidable days, and supporting efficient, patient-centered outcomes. You will collaborate closely with physicians, nursing staff, and post-acute providers to ensure safe and timely transitions of care.

Requirements

• Active California RN License
• BLS Certification
• Minimum 2 years of Case Management or Utilization Review experience in acute care
• Experience with MCG or InterQual
• Knowledge of Medicare, Medi-Cal, and Managed Care
• Strong communication and critical thinking skills

• ICU or critical care background

Responsibilities

• Perform admission and continued stay reviews using MCG or InterQual criteria
• Initiate and manage discharge planning within 48 hours of admission
• Coordinate care across multidisciplinary teams
• Obtain payer authorizations and manage utilization review processes
• Identify delays, barriers, and opportunities to reduce avoidable days
• Facilitate transfers and post-acute placements (SNF, Home Health, Hospice, DME)
• Maintain accurate and timely documentation in the EMR
