Responsibilities

  • Provide direct patient care to Med Surg patients, including comprehensive assessments, monitoring, and timely interventions for acute medical and post-surgical needs.
  • Administer medications, manage IV therapy, wound care, drains, and telemetry, while prioritizing safety, infection control, and evidence-based practices.
  • Collaborate with physicians, case managers, therapists, and ancillary staff to develop, implement, and adjust individualized care plans.
  • Assess discharge needs early, coordinate with families and social work, and facilitate referrals, education, and resources to support smooth transitions home.
  • Document thoroughly in electronic health records, maintain clear handoffs, and participate in rounding, rounds with teams, and quality improvement initiatives.
  • Provide patient and family education on medications, self-care, symptom monitoring, and recovery expectations.
