Responsibilities

Conduct comprehensive resident assessments
Develop and revise individualized care plans
Administer medications with precision and safety
Monitor vital signs
Manage chronic conditions
Assist with activities of daily living
Perform wound care
Deliver IV therapy when within licensure and facility guidelines
Coordinate with physicians, therapists, social workers, and CNAs to ensure care transitions are seamless and goals are clearly communicated
Document thoroughly
Participate in quality improvement initiatives
Respond rapidly to clinical changes or emergencies with composure and leadership
Mentor and supervise support staff
Promote best practices in infection control, safety, and compassionate care
