Travel is required and may include covering Wyomissing, PA to Chambersburg, PA _A company fleet car is provided for eligible employees._ _+ Hospice Case Management Experience Preferred_ _+ Home Care Home Base EMR Experience Preferred_ Would you enjoy making a difference across multiple communities while stepping in where care is needed most? We're looking for a Hospice RN - Regional Support to join our team. In this dynamic role, you'll provide direct care to patients and support to families, while also serving as a floating clinician-temporarily assigned to different hospice branches based on census needs or staffing gaps. Requirements

Current, unencumbered RN license in the state of employment
Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)
Current CPR certification
Valid driver's license, auto insurance, and reliable transportation

Proficient in documentation and electronic health records

Responsibilities

Deliver high-quality, compassionate nursing care for hospice patients
Manage and implement individualized plans of care
Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)
Assess and address patients' physical, emotional, spiritual, and environmental needs
Accurately document all care provided and ensure compliance with hospice protocols
Educate families and caregivers on care instructions, medications, and safety
Participate in the on-call rotation and provide after-hours care as needed
Collaborate with physicians, referral sources, and the care team
Assist with referrals, regulatory documentation, and payer compliance
Travel to various Gentiva hospice locations as assigned to fill urgent needs
