Responsibilities

• Provide comprehensive hospice nursing care in patients’ homes or designated settings, aligning with the approved plan of care.
• Assess symptoms, manage pain, alleviate dyspnea, monitor for changes, and prevent avoidable complications.
• Administer medications under physician orders, adjust plans in collaboration with the medical team, and document all care promptly and accurately.
• Educate patients and families about disease progression, comfort measures, medication safety, and recognition of urgent changes.
• Coordinate with physicians, social workers, chaplains, and other care team members to maintain a holistic approach.
• Uphold infection control, safety standards, and ethical practice; advocate for patient preferences and cultural considerations.
• Contribute to care planning meetings and quality improvement initiatives, ensuring consistent, compassionate care across settings.
