About The Position

Heartline Staffing is a Strategic Talent Partner with Purple Cow Recruiting. We partner with healthcare organizations nationwide to place experienced clinicians in travel and permanent roles. Heartline Staffing is seeking an experienced Registered Nurse – Home Health for a 13-week travel assignment supporting adult and geriatric patients in the Portland, Oregon area. This role focuses on skilled nursing care delivered in the home setting, including start of care, routine visits, recertifications, and discharge planning. Assignment Details: • Schedule: Days 5x8s (08:00–16:30) • Contract Length: 13 weeks • Patient Ratio: 1:1 (approximately 5–6 visits per day; 30 productivity points standard) • Weekend Requirement: No • On-Call Requirement: No • Floating: No • Block Scheduling: Not available • Guaranteed Hours: Facility may call off up to one shift per two-week pay period Compensation: • Weekly Gross: $2,646.00 • Hourly (W2): $35.00 • Weekly Stipend: $1,246.00 This position requires independent fieldwork, reliable transportation, and strong documentation skills. Patients are grouped geographically to minimize drive time, though occasional travel up to approximately 60 miles (60–90 minutes total drive time per day) may occur. Mileage reimbursement is provided at the IRS standard rate. Charting is completed using Epic Remote Client, Rover mobile application, and OASIS. Requirements

Minimum of 1 year of skilled adult home health Registered Nurse experience is required.
BLS certification is required.
Active Oregon Registered Nurse license is required.
Experience providing care in an adult home health setting is required.
Experience completing admission assessments is required.
Experience completing start of care visits is required.
Experience completing routine home health visits is required.
Experience completing recertifications is required.
Experience completing resumption of care visits is required.
Experience with discharge planning, including discharge due to safety or noncompliance, is required.
Experience with wound care is required.
Experience with wound V.A.C. management is required.
Experience with medication administration and medication management is required.
Experience with infection control precautions is required.
Computerized documentation experience is required.
OASIS experience is required and must be disclosed if not present.
Reliable transportation is required.
