In this role, your key responsibilities will center on comprehensive patient evaluation, development and implementation of individualized treatment plans, and ongoing progress tracking to adjust interventions as needed. You’ll conduct initial assessments to determine functional goals and safe discharge plans, reassess progress regularly, and employ a range of therapeutic approaches—manual therapy, therapeutic exercise, gait and balance retraining, neuromuscular re-education, and modalities appropriate to each patient’s condition. Collaboration is essential: you’ll work closely with physicians, case managers, and support staff to ensure coordinated care, timely documentation, and seamless transitions to home or community-based programs. Documentation will be precise and aligned with current standards, ensuring continuity of care across facilities. You’ll also educate patients and families about home exercise programs, injury prevention, fall risk reduction, and activity modification to sustain gains after therapy concludes.
